Understanding Prostate Health and Screening Decisions

If you’re a man over 40—or younger with specific risk factors—a prostate health check is an important part of preventive care. Conditions such as benign prostatic hyperplasia (BPH), prostatitis, and prostate cancer often develop gradually and may not cause early symptoms.

Current clinical guidance places strong emphasis on informed decision-making. This means your GP will explain the potential benefits and harms of prostate-specific antigen (PSA) testing before proceeding, ensuring the decision to test is appropriate for you.

What Your GP Assesses

Your GP will begin with a structured clinical history, including:

  • Urinary symptoms (frequency, nocturia, weak stream)

  • Family history of prostate cancer

  • Ethnicity and individual risk factors

  • General health and life expectancy considerations

PSA testing is discussed in detail, including what the result may and may not indicate. If you choose to proceed, a blood test is arranged.

Importantly, digital rectal examination (DRE) is no longer recommended as a screening test in asymptomatic men in primary care. It may still be used selectively in symptomatic or clinically indicated cases.

PSA Testing: Who and When

Your GP uses evidence-based guidelines to tailor screening:

  • Ages 50–69: PSA testing may be offered every 2 years after informed consent

  • Ages 40–49: Consider baseline testing if you are interested in prostate health

  • Higher risk patients (family history, certain ancestries): earlier and more frequent testing may be recommended (from age 40–45)

  • Ages 70+: Testing is individualised based on overall health and life expectancy

If PSA is elevated, your GP may repeat the test and assess trends before deciding on further steps.

Clinical Reasoning and Next Steps

PSA results are not diagnostic on their own. Your GP interprets results alongside your overall risk profile. For example:

  • Mild PSA elevation may require repeat testing rather than immediate escalation

  • PSA trends over time are often more clinically useful than a single result

  • Additional markers (such as free-to-total PSA ratio) may refine risk

If further investigation is required, multiparametric MRI (mpMRI) is now recommended before considering biopsy. This improves diagnostic accuracy and reduces unnecessary invasive procedures.

Management and Ongoing Care

Your GP will implement a personalised management plan, which may include:

  • Regular PSA monitoring at appropriate intervals

  • Symptom management for urinary concerns

  • Referral to a urologist if imaging or specialist input is needed

  • Preventive health advice tailored to your risk profile

Continuity of care is essential—your GP tracks changes over time and adjusts your plan accordingly.

When to See a GP

You should arrange a prostate health check if you:

  • Are over 40 and want to understand your prostate cancer risk

  • Have urinary symptoms or changes in bladder function

  • Have a family history of prostate cancer

  • Belong to a higher-risk group

  • Want to discuss whether PSA testing is appropriate for you

Even without symptoms, proactive discussion is clinically appropriate.
