Onyx Health Group is a mixed billing medical clinic. The fees applicable to your appointment will be confirmed during the booking process. If you have any questions, please get in touch with our reception team.
New Patient Registration Form
If you are a new patient to Onyx Health Group, please complete our New Patient Registration Form before arriving for your appointment.
You will receive a link to the New Patient Registration Form via SMS or email before your first appointment. If you can’t find it, you can access the link above.
If you’re not yet ready to book an appointment, you can still complete this form and register as a patient, saving time when you make your first appointment.
Bulk Billing Policy
ONYX Health Group is a mixed billing practice.

NDIS participants
Concession Card holders
This model supports access to quality care and promotes health equity in our community.

Cancellation Policy
We understand that your day might not go as planned, and that you may need to cancel an appointment from time to time.
If you need to cancel your appointment, please give us a call as soon as possible, to allow waitlisted patients a chance to be seen.
If you cancel your appointment, our cancellation policy will apply and you will be charged the following fees:
cancellation with less than four hours prior to your appointment time – the total cost of your booked appointment
cancellation with more than four hours notice but less than 24 hours notice – the gap (out-of-pocket) fee for your booked appointment
cancellation with more than 24 hours notice – no fee
Urgent appointments
If you require an urgent appointment, please give us a call and we will try our best to fit you in.
For emergencies, please call Emergency (000) or visit the Emergency department at your nearest hospital.

All appointment fees are time and complexity-based. We provide the best estimate of fees on our website. If your appointment is longer or more complex than expected, a higher charge may apply.
