Recognising When You Need Support

If you’re experiencing persistent low mood, anxiety, stress, sleep disturbance, or difficulty coping with daily life, you’re not alone. These symptoms can affect your concentration, relationships, and physical health. A Mental Health Care Plan (MHCP) is a structured way your GP helps you access appropriate support early, before symptoms become more complex.

What a Mental Health Care Plan Includes

A Mental Health Care Plan is a Medicare-supported pathway that allows you to access subsidised sessions with a psychologist or allied mental health professional. During your appointment, your GP will:

  • Assess your current symptoms (e.g. anxiety, depression, burnout)

  • Explore contributing factors (work stress, family dynamics, medical conditions)

  • Evaluate risk and safety

  • Establish a working diagnosis where appropriate

  • Develop a structured management plan

This is not just paperwork — it’s a clinical assessment tailored to your situation.

How Your GP Assesses Your Mental Health

Your GP uses a combination of clinical interview, validated screening tools, and medical history to understand what’s happening. This may include:

  • Mood and anxiety screening questionnaires

  • Sleep and energy pattern assessment

  • Review of physical health contributors (e.g. thyroid, iron levels)

  • Medication review if relevant

The goal is to identify both psychological and physical drivers of your symptoms, ensuring nothing is missed.

What Happens After the Plan Is Created

Once your plan is in place, your GP will refer you to a psychologist or appropriate service. Typically, you can access up to 10 subsidised sessions per year.

Your GP remains central to your care by:

  • Monitoring your progress

  • Adjusting treatment if needed

  • Coordinating care with other providers

  • Reviewing your plan after initial sessions

Mental health care is ongoing — not a one-off visit.

When to See a GP

You should book an appointment if you notice:

  • Persistent anxiety, worry, or panic symptoms

  • Low mood lasting more than two weeks

  • Sleep disruption affecting daily function

  • Burnout, overwhelm, or emotional exhaustion

  • Difficulty coping with work, study, or relationships

Early assessment leads to better outcomes and more straightforward management.
